Applications must be submitted by Tuesday, July 1, 2009 to be considered on time. All submittals will receive a confirmation e-mail.

Tournament applications are completed over the Internet only.
- If you have applied to a tournament using GotSoccer you can use your Username and Password to login and use your team information to apply to the 6th Annual Park City Extreme Cup. Otherwise you will need to go in and create a free account with GotSoccer for your team by proceeding through this application.
- Click on the link above to begin your application. Once you create your team’s account you may save the data and return as often as needed to complete the application. Using your Username and Password, you may also return as often as you like; GotSoccer offers great team pages and email capabilities for FREE.
- You must register for the age group your team will play in during the fall of 2009.
- Application fees can be paid either by credit card or by mailing a cashier’s check or check. Checks should be made payable to Park City Extreme Soccer Club.
- Each page in the application has instructions you need to read. Please remember to be thorough.
- You can access this application form 24 hours a day, 7 days a week.
- The information requested in the application regarding last years tournament results for the Park City Extreme Cup is not required, but we would like as much information as possible for scheduling and seeding purposes.
