Team Check-In

All teams must attend an in-person tournament check-in before being allowed to participate in the Park City Extreme Cup. This mandatory meeting will be held on Tuesday, August 3rd, 4:00-8:00 pm at Newpark Hotel (Google map). Teams traveling from out of state or more than 2 hours drive from Park City are eligible for special check-in arrangements. Any special check-in requests MUST be arranged at least a week before the tournament and approved by the tournament director.

Documents Required at Check-In:

  • Two (2) copies of your “official” team roster.  This is the roster that is signed by your district registrar and comes with your player cards.
  • Five (5) copies of your “tournament” roster. This is the roster of players attending and playing in the tournament, including guest players. If you have a tournament roster with your GotSoccer team account, you may submit printed copies of that. TOURNAMENT ROSTER FORM
  • UYSA player cards for all players, including guests (out of state teams – the player cards from your state association).
  • UYSA player participation forms for all players, including guests (out of state teams – the form from your association). This form includes consent for medical treatment.
  • Birth certificates for all players (UYSA requirement for 2010). We do not need to keep these.
  • Guest player form for all guest players. Guest players also need a UYSA player card and participation form. The guest player form can be found at:  GUEST PLAYER FORM
  • Current coaching cards for all coaches.
  • Travel papers for all out of state teams.

If you do not have all of the required documents at check-in, your team will forfeit games in the tournament.

During the check-in process, your 5 tournament rosters will be stamped for official game use. Bring one copy of this roster to each game played.
